Electronic communications may involve certain risks. Information transmitted by form, email or SMS should be limited to what is necessary for the request.
Information collected
We collect the information needed for your care: identification, contact details, medical and dental information relevant to the safety and quality of care.
Use of information
Your information is used to organize appointments, maintain the clinical record, deliver care, follow up and communicate as needed for your care.
Sharing of information
Your information is only shared when necessary for care, for a referral to another professional, for collaboration with the laboratory, or when required by law.
Security
Reasonable technical and organizational measures are in place to protect your information against loss, unauthorized access, modification or unintended disclosure.
Access and correction
You may request access to your information or correction of inaccurate details. An identity check may be required before processing the request.
Consent
Your consent is required to transmit certain information or documents (specialist referral, transmission to the laboratory, electronic communications). You may withdraw your consent for purposes not essential to care.
Electronic communication
Communications by email, SMS or web form may involve risks inherent to digital transit. The information transmitted should be limited to what is necessary for the request. For particularly sensitive information, an in-person or phone exchange may be preferable.

For any question regarding privacy, access to or correction of your personal information, or data security, you may contact the De Facto Dental Studio's person responsible for the protection of personal information by phone or email. De Facto Dental Studio operates under the laws applicable in Quebec and Canada, in particular the Act respecting the protection of personal information in the private sector (Law 25) and PIPEDA at the federal level.
